This post will go over everything you need to do in order to fix your lag (specifically for FF14, but will work for other games). There are many different kinds of lag, we will go over all the kinds of lag you can fix, and how to fix them. These aren't in any particular order, and some require more technical knowledge than others, so be sure to check out all the fixes and use the ones you can manage to use. If you feel uncomfortable applying any of these modifications, just skip over them (some require you to enter commands in command prompt).

First off, unless you've tried Titan HM / Titan EX and similar fights where you really need to start dodging stuff you may not even realize that you are lagging. Think about it like this: if Titan EX plumes aren't ridiculously easy to dodge, then you are lagging.

Rerouting: If you've already covered everything you can do within your network, let's cover what you can do outside it. This may be the best fix out there if you are lagging, have already tried everything you possibly can, and have no idea why your usually great internet sucks for FF14.
The FF14 servers are often behind ISP routers that are faulty, slow, and congested. Dependent on your ISP and physical location, you could be routing through one of these routers which can lag you and make you unable to do anything about it, except use WTFast. I have personal experience with this and Verizon Fios in the northeast US.
WTFast is a program that you can run on your computer and set up to specifically take your FF14 (and other gaming connections) and tell them to route through the faster WTFast ISP routers instead of the awful laggy routers your ISP goes through.
Download, install, and run WTFast selecting FF14 as your game. Sort the servers you can connect to by ping and choose the one with the lowest ping and go.
Note: WTFast has a 30 day trial, after that a standard membership is $5 / month, and I consider this well worth it to fix otherwise unfixable lag. The pricing is pretty reasonable, after all you are routing through their servers. Crusader kings ii soundtrack.
An alternative to WTFast that seems to do the same thing is PingZapper, Looks to be cheaper than WTFast monthly as well, although they seem to have fewer servers to connect to so WTFast may still be the better option for $2ish more a month.

Go wired: If you are on wireless: get off of it and plug in a wire. Wireless usually provides you with an unstable intermittent connection that is especially bad for the way FF14 treats packets. To test this, open a command prompt and type:
then find your Default Gateway IP Address (usually 192.168.1.1, 192.168.0.1, or 10.0.0.1 for most standard routers)

This will perform 200 pings to your router. If at the end of the 200 pings, your ping to your router has ever gone any higher than 10ms, congratulations your wifi is laggy. Actually even 10ms is higher than you'd be getting wired because a wire will consistently give you < 1ms. Even if you do only get something like a 20ms fluctuation to your router, don't think that its negligible. That extra 20ms is just to your router, FF14 is going to lag a lot more while its waiting for you and your router to work out your differences.
Use a Streamboost Router: If you have roommates or other people using your router, your gaming is likely often a victim to their skyping, heavy video streaming, and torrenting. Get a router that has Streamboost which automatically prioritizes your gaming over other connections in a way that is much more effective than QoS on other routers (even if you know what you are doing and customize QoS on other routers, Streamboost is still better). I recommend the Dlink DGL-5500 since I'm unaware of any other Streamboost routers currently on the market. Make sure to set up your gaming computer to have the highest generic priority within the router.
With the DGL-5500 you can also see what kind of bandwidth the different devices on your network are using, as well as what types of programs are using that bandwidth (you can see who is torrenting and how much bandwidth their torrenting is using).
Disable power saving on your network cards: Right click My Computer / Computer and select Manage. Go to Device Manager and then Network Adapters. Right click the network adapter you are using and select Properties. Go to the Power Management tab and uncheck the checkbox there, preventing your computer from using power management on the device. You may have to disable / re enable your network card, or restart your computer to get this to actually work.
Fix Windows 8 Networking: Due to the way Windows 8 sends packets your networking is inherently unoptimized for games like FF14, but its an easy fix here's how you do it. Note: This fix is for the most part negligible, but can give you a slight advantage in certain situations by causing Windows 8 networking to operate more like Windows 7. Only do this fix if you know what you're doing. To reverse this fix you have to uninstall and reinstall your network card (delete the device and reinstall it), so only do this if you know what you're doing.
Open an elevated command prompt: go to Search (the right bar), type in: cmd, and when you see 'Command Prompt' right click it and select Run as administrator.
In the elevated command prompt paste this code, one line at a time followed by hitting Enter after each line you paste (ctrl+v wont work, you must use right click->paste)
If you get an error/warning message after pasting this code in line by line and hitting enter after each line, don't worry about it, you're supposed to get one. Disable/re-enable your network card, or restart your computer for the changes to take effect. Congratulations you've fixed the shoddy Windows 8 networking. This will simply make your connection to games a bit more stable, and cause your back-end Windows 8 networking controller to operate more like a Windows 7 networking controller.
Leatrix Latency Fix: Download and run Leatrix Latency Fix as an administrator (right click, open as administrator), apply the fix, disable/re-enable your network card or restart your computer, and then forget about it. It will optimize the way your network card sends packets, making it more in line with the way FF14 and other MMOs like to receive them, decreasing your lag. NOTE: If you have an older router, or a router that can't handle an increase in the number of packets it is sending/receiving then Leatrix Latency Fix may cause your router excessive stress thus slowing down your internet. If you notice this happening you can simply disable Leatrix Latency Fix by running the program again and clicking the button to turn it off, and then disabling/re-enabling your network card or restarting your computer.
